Elder Care Management
Rehab Assist provides elder care management services to those elderly individuals who are compromised either mentally or physically and who may require monitoring, supervision, guidance, or planning. Services include assessment of needs, creation of a care plan, life planning, monitoring of care, and budgeting and coordination of services. Rehab Assist may also coordinate physician appointments or community activities. A “hands on” personal approach is utilized at all times.

Guardianship Services

This service monitors and oversees the individual's mental, physical and emotional status. This is achieved through onsite monitoring and advocacy for the client. Issues regarding appropriate residence, medical needs, educational needs and vocational placement, to name a few, can be addressed. Reports are made to family members and designated advocates on a regular basis. Nursing homes, rehab units, school environments and homebound clients are frequently visited to assure proper care.
